Case Management Services

Aztecs Rising provides case management services through its gang intervention services and the Junior Aztecs Fire Fuels Program.

Case management services are offered to youth and adults who are high risk, gang involved, gang affiliated, have committed gang related crimes or have the potential to join a gang. Recruited through street outreach, probation officers, law enforcement agencies, schools and local community based organizations; clients receive comprehensive case management that includes individual and family counseling, family mediation, anger management and job training and placement. Upon being enrolled to receive gang intervention services or into the Junior Aztecs Fire Fuels Program, clients are assigned a case manager who assists in developing an individualized service plan that reflects each client's needs. Depending on the client's circumstances, the plan could include a number of action steps required, such as home and school visits from the case manager, attendance at anger management and life skills workshops, personal goal planning or other support services.

Youth receiving case management services have the opportunity to be involved in the Junior Aztec Fire Fuels Program (Junior Aztecs). The Junior Aztecs teaches youth environmental care, team building and communication skills through personal development workshops, physical training, outdoor recreational activities and community service activities. The Junior Aztecs program provides building blocks of self-esteem and positive choices for youth of Los Angeles' toughest neighborhoods. Additionally, youth on probation have the opportunity to complete their probation requirements through the Junior Aztecs.
